The 2021-2024 Community Health Improvement Plan (CHIP) is a plan for action that defines how our community can work together to address priority health issues identified by Eau Claire County residents in the Community Health Assessment
Healthy Communities, the Alliance for Substance Misuse Prevention, and partner organizations across multiple sectors of our community will use this community-owned plan to direct programs and policies to create a healthier Eau Claire County for all. The County Health Rankings and Roadmaps Take Action Cycle was used to guide the CHIP process. The process is outlined below.

The CHIP relies on the resources and collaborative efforts of a wide range of stakeholders. It reflects the work of many dedicated people and organizations who are working to improve health across our county.
Over the next three years, Healthy Communities, the Alliance for Substance Misuse Prevention, partner organizations, and residents will implement evidence-based practices and evaluate the community’s progress toward improving our health priorities. The goal is to make Eau Claire County a place where everyone can attain health.
