Community Health Improvement Plan

The Community Health Improvement Plan (CHIP) is a working document created in order to help guide the work of all Eau Claire Healthy Communities’ action teams. This planning process heavily considers the needs of the community and relies on community input to produce results. Eau Claire Healthy Communities utilized the County Health Ranking and Roadmaps: Take Action Cycle to guide the planning process which is outlined below.

Chip Process

The CHIP provides action teams with a frame for improving the health indicators that were prioritized by Eau Claire County residents in the Community Health Assessment. It reflects the collective work of many dedicated individuals and organizations who are working to improve health outcomes for all of Eau Claire County.

Over the next three years, Healthy Communities’ action teams will continue evaluation and implementation of evidence-based practices and policies in order to reach their goals. Efforts will be updated to align with community resources and needs as necessary.
